This is a SEAS 2 Grow partner, involved in the project activities on a daily basis.
Real-time digital handover system comprising of applications to enable end-to-end connectivity between systems and formal communication between clinical staff. It currently consists of Emergency Referral, Community Nurse Referral and a mobile messaging platform. The vision is to grow the system to include other applications that would be used in the care environment when accessing care and patient movement.
